When I open Outlook the OfficeCalendar icons have disappeared. How do I get them back and why did they disappear in the first place?.

OfficeCalendar icons do not appear within Microsoft Outlook 2010/2013/2016 or Office365
OfficeCalendar icons do not appear once Microsoft Outlook is started for Office 2007
OfficeCalendar icons do not appear once Microsoft Outlook is started for Office 2000/2003
